We can help a variety of individuals of all ages. Our client's ages have ranges from 28-98. Many of our clients have been veterans of WW2, the Korean War and Vietnam. Some of our clients emigrated from other countries, which spoke very little English, so we are committed and determined to meet everyone's needs. Our experience is not only with physical conditions, but also mental health disorders.
We have many clients with schizophrenia, bipolar disorder, personality disorders and depression to name a few. Some of our clients have Alzheimer's disease, dementia, substance abuse histories, alcohol abuse and behavioral/mood disorders, developmental disorders, and many other issues, as we do not discriminate those who need our help. Our goal is to make you and your loved one's life easier through trust, integrity, compassion and respect.
Oftentimes, we work directly with care facilities, hospitals, physicians and other professionals to provide monitoring of our clients care needs on a weekly, bimonthly or monthly basis. Our case managers provide a liaison for care facilities to obtain orders from physicians for the continuity of care for each of our clients and provide all the necessary documentation.
Families and advocates who have difficult decisions to make can also benefit from a sympathetic and understanding case manager. We have the knowledge and skill to help families make the right decision for their loved one. We strive to give you the answers you need with the tools to evaluate most all situations, through experience, determination and creativity.
Financial planners, trust officers, attorneys, guardians and conservators can also benefit from our case managers by monitoring their clients needs, assisting with needed documents and providing routine visits to assure their clients are receiving the best care possible.
